Jockey Club C • Care Programme - St James’ Settlement Care with Love Support Service
Introduction
Thanks to the generous donation from The Hong Kong Jockey Club The Charities Trust, our organization has cooperated with special schools on Hong Kong Island to implement a three-year school-leaving support service. By assessing family needs to plan to provide targeted support services to help carers cope with daily challenges, relieve caregiving pressure and enhance family resilience, promote school leavers to adapt and smoothly transition to adult services, and build a caring and mutual-help community.

1. Carer Support Service
- Psychological intervention
- Caring stress management class
- Physical health promotion programme
- Caring skill training
- Personal support building
2. Support Services for School Leavers
- Home-based training ( only available to service users who meet specific qualifications; carers need to participate in training at the same time )
- Escort ( only available to service users who meet specific qualifications; carers need to participate in the escort at the same time )
- Life skills and self-management training
- Adaptation programme for adult services
3. Establish support network and community education
- Seminars for parents of special schools
- Community education activities
- Community resource matching
- Volunteer training
Service Target (People with Intellectual Disabilities)
- People with intellectual disabilities who are studying or graduated from schools on Hong Kong Island and their carers
Fees:
Free of charge
( All the services provided by the St James’ Settlement Care with Love Support Service ” are free of charge, but the personal expenses incurred during the service flow, such as lunch expenses, transportation expenses, registration of services/activities provided by other organizations, all fees must be paid by the service user. )

Referrals / Application:
Applicants can approach the operating agencies directly or referral by social workers

Monday to Friday 9:00am-5:00pm
(except public holidays)
